The intermediate syndrome presents as a complication secondary to organophosphate intoxication, was described in 1987 and starts once they have resolved the symptoms of cholinergic crisis, manifested by proximal muscle weakness and diaphragmatic prolonging the need for support ventilatory and brings greater number of comorbidities. We report a patient who was referred to the Pediatric Intensive Care Unit, University Hospital of Neiva Hernando Moncaleano Perdomo in the presence of respiratory failure, hypotension and bradycardia after voluntary ingestion of organophosphates, diagnosis was made of "Intermediate Syndrome" and literature review was conducted about.
